Posterior Urethroplasty / PFUI Repair
Posterior urethroplasty for pelvic fracture urethral injury (PFUI) addresses the distraction defect between the membranous or bulbar urethra and the bladder neck created by blunt pelvic trauma. The repair is typically deferred 3–6 months post-injury to allow hematoma resolution, and uses a perineal approach with progressive steps of urethral mobilization, separation of corporal bodies, and inferior pubectomy as needed to achieve a tension-free anastomosis. Success rates exceeding 90% are reported in experienced centers using the elaborated anastomotic technique.
